چکیده

A 77-year-old female patient with sinus bradycardia and left bundle branch block was implanted with a DDD Biotronik Kairos DR pacemaker in 1999, then a replacement for depleted battery was performed in 2007 (Biotronik Axios DR). During our regularly scheduled follow-ups, we found the atrial and ventricular sensing/pacing function to be satisfactory: P: 3.6 mV, R: 9.9 mV, atrial threshold: 0.7 V/0.4 ms, ventricular threshold: 0.5 V/0.4 ms. Parameters were set as follows: basic rate: 60 bpm, hysteresis: −10 bpm, dynamic AV delay: 225 ms, sense compensation: −45 ms, atrial sensitivity: 1 mV, and ventricular sensitivity: 5 mV. During the latest follow-up (14 years after the first implantation), the following recording was made (Fig. 1). What would be your interpretation of this recording? Answer You will find the answer elsewhere in this issue.

Abstract: Background and Aim: Since electrocardiography (ECG) is essential for the diagnosis of pediatric asymptomatic Complete Heart Block (CHB) and it is not usually done in the preoperative evaluation of asymptomatic children, physicians might confront with asymptomatic cases of CHB at anesthesia induction. Case Report: In this study, we introduced a 31-month-old boy who faced CHB after anes...

Investigation of the BNP Level Changes in Blood Stream in Different Modes and Lead Locations after Pacemaker

As has been proven, increase of mechanical strain could result in an increase of brain natriuretic peptide (BNP) in the blood stream of implanted patient pacemakers. We measured the BNP concentration in blood due to different mode and lead implantation location of pacemaker in the time period of 3 months. The aim of this study was to investigate the changing trend of BNP level after pacemaker i...

Adverse effect of ventricular pacing on heart failure and atrial fibrillation among patients with normal baseline QRS duration in a clinical trial of pacemaker therapy for sinus node dysfunction.

BACKGROUND Dual-chamber (DDDR) pacing preserves AV synchrony and may reduce heart failure (HF) and atrial fibrillation (AF) compared with ventricular (VVIR) pacing in sinus node dysfunction (SND). However, DDDR pacing often results in prolonged QRS durations (QRSd) as the result of right ventricular stimulation, and ventricular desynchronization may result. The effect of pacing-induced ventricu...

Effective Management of Atrioventricular Interval for Paroxysmal Atrial Fibrillation That Developed After DDDR Pacemaker Implantation in a Sick Sinus Syndrome Patient.

A 68-year-old man with sick sinus syndrome (SSS) was referred to our department for pacemaker implantation. After implantation of a pacemaker with rate-responsive dual chamber (DDDR) mode and minimized ventricular pacing (MVP) functions, paroxysmal atrial fibrillation (PAF) repeatedly developed. Pacemaker memory showed that the intrinsic atrioventricular (AV) (atrial pacing-ventricular sensing ...

New Concepts in Pacemaker Syndrome

After implantation of a permanent pacemaker, patients may experience severe symptoms of dyspnea, palpitations, malaise, and syncope resulting from pacemaker syndrome. Although pacemaker syndrome is most often ascribed to the loss of atrioventricular (A-V) synchrony, more recent data may also implicate left ventricular dysynchrony caused by right ventricular pacing. Previous studies have not sho...
